About the Opportunity
We currently have an opportunity for an Inpatient Nurse Practitioner to join our team in Toronto, ON, on a full-time basis.
Hours: 37.5 hours/week, including Thursday and Saturday, but flexible schedule otherwise.
As an integral member of a multidisciplinary clinical team, the Nurse Practitioner (NP) will work collaboratively with physicians, unit leaders and the broader clinical team to provide trauma-informed clinical care to Casey House inpatients. The NP will be responsible for: admissions to and discharge from Casey Houses inpatient unit, autonomously assessing, diagnosing; ordering and interpreting diagnostic tests; prescribing pharmaceuticals and performing specific procedures within the legislated scope of practice; to ensure a timely response to a patients changing health status and needs; and providing direct patient care focusing on health promotion and maximizing patients' safety and function. Actively participating in regular team meetings, liaising with other hospitals and community-based providers, advocating on behalf of patients and their families and participation in inter-professional patient and family conferences as needed.
This is the first NP role for Casey Houses inpatient unit as part of an exciting initiative to streamline patient flow processes and improve continuity of care. The successful candidate will have a role in both developing intake and outreach processes and making improvements and modifications as it progresses. This is a great role for someone who is excited to take on a highly visible leadership opportunity.
More specifically, your responsibilities in this role will entail, but are not restricted to:- Reviewing referrals and assessing patients to identify appropriate patients requiring hospital admission to Casey Houses inpatient unit and facilitate admission in a timely manner
- Providing comprehensive health care to patients living with or at risk of HIV
- Facilitating transfer of care to the primary health care provider and community resources when the patient is discharged from hospital
- Integrating and promoting excellence in clinical skills and knowledge within a broad scope of advanced nursing practice through direct patient care, program design, clinical consultation, staff education and quality improvement initiatives
- Working within a multidisciplinary team, including physiotherapist, recreation therapists, massage therapists, mental health clinicians and social workers to provide holistic compassionate goal-driven care to Casey House patients
- Liaising with the Chief Medical Officer, Outpatient NP and team to provide seamless transitions between the Inpatient and Outpatient unit
- Supporting the evolution of a model of care to meet the needs of a diverse and complex patient population
- Promoting and supporting a climate of continuous quality improvement and clinical excellence
- Participating as a member of the Casey House Leadership Team
- Acting as an expert resource for trauma-informed primary care provided within a harm reduction framework
- Providing leadership in advancing clinical practice and the provision of safe and high-quality patient-centered care
- Participating on teams and external committees to plan, coordinate and deliver new and existing services, communicate information, resolve problems, and achieve the goals of the unit
- Advocating for patients/clients, to protect and promote their rights to dignity, respect, privacy, confidentiality, autonomy, and access to information
- Role-model practice consistent with Casey Houses values and philosophy of care
About You
In order to qualify for this role, you must have a current CNO registration as a Registered Nurse in the Extended Class (RNEC), with a specialty in either Adult or Primary Care. You must also have a minimum 2 years working with clients with mental health or addiction issues, with demonstrated commitment to, and knowledge of, harm reduction and anti-oppressive framework of practice. Preference will be given to candidates who possess, or are currently pursuing, a Masters in Nursing or related graduate degree.
The following qualifications will ensure your success in this role:- Successful completion of a Controlled Substances Prescribing course
- Current BLS for Healthcare Providers certification
- The ability to work independently and as a member of an interprofessional team, and a demonstrated ability to carry out the responsibilities of the role effectively and professionally is required, including demonstrated ability to establish and maintain effective therapeutic relationships with complex patients
- Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively. Comprehensive understanding of community-based services and supports which address issues of poverty, gender equity, homelessness, marginalization, mental health, and substance use
- Extensive knowledge of the issues arising from disabilities, deteriorating mental/physical health and of the conditions which contribute to social isolation, discrimination, and racism

Reporting to the Director of Care or designated clinical leadership team member, the Personal Support Assistant helps with select nursing and personal care tasks in accordance with residents plan of care and, which are within their knowledge and skill, while adhering to regulatory and Reveras standards, policies and procedures. Job Description:Registered Respiratory Therapist ( Full Time 12-month contract) - Windsor, ON Primary Purpose The Healthcare Professional is responsible for initiation of prescribed respiratory therapy in the office, or the client home including assessment, education, documentation, and follow-up to ensure equipment performance and client compliance. Promotion of products and services to referral sources, including educational in-services to medical/healthcare communities is an integral part of the job. Key Accountabilities * Initiate and monitor of prescribed respiratory therapy in clients' homes including assessments, education, documentation and follow up to ensure equipment performance and client compliance with therapy * Delivery and exchange of equipment in client's homes may include heavy lifting of equipment up to 60lbs * In home client visits may include long distance travel * In office consultations with sleep and oxygen clients * Conduct respiratory equipment in-service training with various community partners * Promotion of products and services to referral sources. * Build and maintain high quality client and referral relationships * Perform all job duties in compliance with Safety standards; * Support in the training and development of new employees as needed * Participate in a regular on-call rotation Required Qualifications Education * Registered Respiratory Therapist with current registration Experience * Must have CPAP training and experience * Minimum 2 years related experience preferred * Homecare and sales experience an asset Knowledge / Skills / Abilities * Excellent customer service and communication skills * Ability to prioritize and multi-task * Attention to detail and results oriented * Able to work under pressure and have strong sense of urgency * Must possess commitment to client and personal safety * Maintain client confidentiality * Clear Driving record/abstract At Medigas, the healthcare division of Linde Canada, Inc., you'll join talented and diverse people throughout Canada, all working hard to make a difference in our client's lives and make our organization successful.
